मी, याद्वारे घोषित करतो की, महाराष्ट्र कामगार कल्याण मंडळाच्या आर्थिक लाभाच्या योजनांसाठी मी अर्जात नमूद केलेली माहिती व सोबत जोडलेली कागदपत्रे माझ्या व्यक्तिगत माहिती व समजुतीनुसार खरी आहेत. जेव्हा आवश्यक असेल तेव्हा यासंदर्भातील मूळ कागदपत्रे सादर करण्याचे मी वचन देतो. सदर माहिती व कागदपत्रे खोटी असल्याचे आढळून आल्यास भारतीय दंड संहिता अन्वये / किंवा संबंधित कायद्यानुसार मी कायदेशीर शिक्षेस पात्र असेन.
